Why can't I hear from the ear speaker on my iPhone 12 Pro?
Every time a call comes in or out. Have ta use speaker phone my earpiece either is turned down or off.

Try disconnecting and repairing the earpiece. Make sure to forget it in Bluetooth. Or maybe reset it, if it has that option. I suggest you look for a user manual for the earpiece, as those often have great troubleshooting steps.
If the earpiece has an app, troubleshooting steps can often be found within the app.
If you can’t find the manual, consult with the manufacturer of the earpiece, and explain to them what going on.
