How do I stop ghost notifications with bling sound on iPhone 16 Pro Max?

My phone is on silent. Focus mode. I keep getting ghost notifications. My phone vibrates with a bling sound (even though it's on silent). I've checked Screen Time > Notifications to see what apps are sending me notifications, and there's nothing there either. The notification sound reminds me somewhat of the old MSN messenger bling sound. It happens twice or thrice a day I think.

So what's going on?

My phone is iPhone 16 pro max. Updated to latest OS (iOs18.3.2)
