How do I restore the missing flashlight icon in Control Center on iPhone?

Swipe down the Control Center and look along the right edge. Tap the different icons and look for your missing Flashlight icon.

Swipe down from the top right to access Control Center, tap the + in the top left, tap Add a Control in the bottom, search Flashlight and tap it to add it back.
