Ahhh-- your signature indicates that you are running macOS 10.13 High Sierra, so I had assumed an older Mac--apparently that's not correct.

In your initial post you wrote "running Sequoia Version 15.3.2 from Spain".

Have you installed the most recent updates by now?

For me (in Germany, also in the EU) the clean-up tools vanished from my Mac with Sequoia Version 15.3.2 and only came back after the update to Sequoia Version 15.4.1. So I would suggest to ensure that the most recent update has been installed.

My best guess is way too advanced for Apple’s current tools. 😢

I don't know if you can cleanly remove someone from a photo with the Clean Up feature. Why not try yourself. If it doesn't work our as expected you can always revert back to the original.

I've removed people from a photo with the Clone tool of Photoshop Elements (the top photo):

It really depends on the image. If the person or object to be removed is relatively small and/or isolated from the main subjects, the Photos does a respectable job. Compared to what Adobe is doing, Photos is really lagging behind the major photo editors.

Are there other people in the photo? How close to them are you?

I did this quickly as I was putting together my reply. It's very rough but better than I expected. I used the "Erase" mode of Clean Up. The Retouch tool made a mess.
