How can a hidden photo on iPhone be visible when attaching a picture in iMessage?

When you hide a picture in Photos, the app doesn't go around to all your other apps hunting up the picture to "hide" in those, as well. It's good to know that if you edit or delete a picture in Photos, it will not be changed or deleted in Instagram or Messages Photoshop or other apps you may have.

I may have misunderstood-- I took you to mean that the picture had been in Messages before you dealt with it in Photos.

Do you mean that you mark a picture as Hidden in Photos, then go to Messages and hit the + button to add a picture, choose Photos, and you see the picture? -- That doesn't happen on my iPhone. If I hide a picture, then try to add a picture in Messages, the hidden picture is not a choice.

Which situation did you mean?
