I really wanted to the Japanese version of a popular rhythm game, but I'd need to change my region for that. If I'm signed into an iCloud account on both devices, and change the region of one device, does it affect the other device?

The device region is not linked to the App Store, you would need to change the iCloud / Apple Account's region for that, and yes, it will affect all devices logged in to that account.
You will also, need to cancel any and all active subscriptions, pending purchases, preorders etc.. as well as spend any and all Apple Account balance you may have before you can change the country.
