How do I remove a persistent microphone icon on my iPhone screen?

When i tried to click the icon it shows microphone and my name to listen. I tried turning on/off my phone but it's not disappearing. It's just on my pocket and after opening my phone the icon pops up and i can't remove it. Can you guys please help me!
