If only the back glass is broken and there is no other damage, Apple can replace just the glass on the back of your iPhone. Katana-San provided a link where you'll see that with AppleCare+ it will cost $29 to replace the back glass.

To be clear, AppleCare is the standard one year warranty provided on every new iPhone. It does NOT cover accidental damage. AppleCare+ can be purchased within 60 days of purchase of an iPhone.

Depends. Since the back glass is broken, they will probably give you a replacement phone, but they will determine the best course of action for your problem.

Make sure you make an appointment at the Apple Store before you go, so you don't have to wait around.

AppleCare won't cover accidental damage to the phone.

If you signed up for AppleCare+, the phone will be repaired or replaced for a $29 fee.

I believe AppleCare is no longer available and now refers to the standard one year warranty. Plus is the only AppleCare being offered.
