A strange issue has started on my iPhone which is affecting my productivity flow. A few days ago my apps were suddenly organized into random folders - except for the ones on the home page of my iPhone screen which remained as app icons. A few hours later, everything was back as it should be. The phone has done this multiple times in the past few days. Any suggestions are much appreciated. Thank you.

The automatic re-creation of this is a bug. So,...
Report this as a Bug to Apple:
Do your Part: Provide Apple with feedback on this, letting them know about this issue. Apple may not get back to you directly, but the more feedback they receive on this, the more they will know what is occurring, and what bug fixes to include in updates to come. Apple will know nothing of your occurrences, if you don't report it to them.

Check for Profiles:
See if there are any profiles on your iPhone. If you do not need them, then remove them. Profiles would make things be arranged and configured in a certain manor: Install or Remove Configuration profiles on iPhone – Apple Support
