Can I create a custom route in Maps?
Specifically, can I alter the route that Maps automatically provides?
If it is possible, can I then send it to my iPhone as amended?

Apple Support provides a user guide online for Maps:
The "Table of Contents" on the welcome page leads to "how to" articles on what Maps can do.
Maps User Guide for Mac - Apple Support
Some of the articles in the guide say: "Note: Not all features are available in all countries or regions".
So what Maps can do for you may depend on what country you are in:
