I may sound silly but when the iPhone isn't on speaker it's designed only to be heard when brought up to your ear.
But if that's what you are doing, go to Settings > Battery > Battery Health and Charging. Because higher volume will drain more battery, a significantly degraded battery can cause the volume to not output as loudly.
