It’s a software bug for some iCloud accounts. I tried resetting and restating over 10 times across three devices and two iCloud accounts.

Just got IPhone 17 pro max with Ultra watch 3 both have ios26 updated. Over 22, no history of hypertension and I live in the US, NE Region. When I go into Health Checklist I don’t even see nor have the option to select the hypertension setting, it’s just not even there, why?

I too do not see hypertension notifications within the health checklist. I have the iPhone 15 pro max and ultra watch 2. However it did initially appear once and I selected over 22 and no prior history and after selecting continue the window disappeared from the checklist!

I have same issue , i have Apple Air and Apple watch Ultra 3 , and my Apple ID US and my Esim is US as well and it gave me , hypertension notification are not available for your region ! im aboard now not in USA but should be working .. please advice

It resolved on its own later the same day for me. Didn’t do anything to fix it.

Same issue for me. Series 11 and 16 Pro. Both on iOS 26. I completely wiped health data. Hypertension Notification is not visible.

Only had the issue on my Series 10 – my Ultra 2 worked as soon as it was updated to os 26.
