On your home screen, swipe from right to left till you arrive at the App Library. Look for the app, then hold you finger on it till the apps jiggle and slide it back to whichever home screen you want it on.

You should be able to find it in the App Store where there will be an iCloud icon next to it to redownload.
You might even have accidentally slid it to the homescreen next door.
