Cannot exit from Assistive Access. triple click not working

setting up an iPhone SE for my elderly mother, Assistive access was working fine and exiting with the home button clicked 3 times. Then after I changed a couple of settings I can no longer exit with the home button clicked 3 times, or the side power button, A forced reboot also does not fix the issue. Now it's stuck and most of the apps just crash when trying to exit with the home button.

I think I turned on Accessibility Shortcut and Unlock with One Tap and Hide Back Button for Unoptimized Apps.


Is there a way to force it to exit Assistive Access ?

Wondering if Apple Support's online article (link below) can help:

Enter and exit Assistive Access on iPhone - Apple Support

Eg: says:

"If you forget the Assistive Access passcode, you can use your Apple Account to exit Assistive Access and reset the passcode. See Change the Assistive Access passcode."
