Sooner or later, you are going to need to make an appointment at the Apple Store / Genius Bar or take the phone, cable, power supply etc, in to an authorized Apple service shop to have everything checked and tested.

Issues with Power Charger:
Whats the source of power? If using a charging brick, what’s the wattage of the charging brick? 20 Watts is the minimum recommendation to go for one: Power adapters for iPhone - Apple Support
