My iPhone SE 3 generation suddenly has begun to give the caller a busy signal, when I have "do not disturb" turned on - can anyone help with how I get back to that the caller hear normal ring tone, when I turn "do not disturb on"?

That's being done by your cellular carrier, not your phone. Your phone is immediately rejecting the call when it is on DND. Because you don't have voicemail, the caller will get a busy signal.
If you don't want that to happen, have the voice mail turned back on.
DND should route the call to voicemail. Voicemail is a carrier feature. There seems to be an issue with your carrier's voicemail system. contact them.
You need to work with your Carrier on this. Your phone is only the conduit for whatever service your carrier provides. You have complicated this by insisting on not having voicemail. But that was your choice. If someone close to you has a emergency and needs to reach you. Even if your phone is not on DND, they can't leave a message.
Since you don't have voicemail, when you have DND turned on, a caller can't leave you a message. So I'm not sure why it would matter to you if your phone rings busy to a caller or not? They can't get a call through because you don't have voicemail. Do as KiltedTim suggests if you want this behavior to change.
Thank you - but the voicemail has been disconnected for years and I talked to the carrier today and nothing has changed. Until last week - when do not disturb was on - calls was just silenced, so the caller heard a normal ringing tone. Now unfortunately the call are rejected
Thank you - I just would like it to be as it has been the last years:-)! And when callers get a busy tone they keep calling, because they think I am with my phone - so getting back to the caller just getting a normal ringing tone is my wish.
Just half an hour ago I called my number with another phone and I just got the normal ringing tone maybe three times and then it rejected. When I called again it rejected immidiatly again and again.
