In general, AppleCare+ does not cover cosmetic damage as long as the phone is still functioning, but the final decision will be made by Apple or an authorized Apple service shop.
Make an appointment at the Apple Store / Genius Bar or take the phone to an authorized Apple service shop to have them review your case with you.
