Troubleshooting the iSight Camera:
If my previous reply is of no success, (I.)report this to Apple, and (II.)get this serviced:
I. Reporting this to Apple:
Do your Part: Provide Apple with feedback on this, letting them know about this issue. Apple may not get back to you directly, but the more feedback they receive on this, the more they will know what is occurring, and what bug fixes to include in updates to come. Apple will know nothing of this, if you don't report it to them.
II. Get your iPhone Serviced:
If you've had this issue since you've owned the iPhone, then it seem you have a faulty device. So, get the serviced, seeing if the solution all comes down to getting a new iSight Camera.
A. Take to an Apple store:
You'd setup a Genius Bar Appointment at your local Apple Store, taking it in for service. There, it will be diagnosed, and you'll be provided with your options.
Or...
B. Take this to an AASP:
See an AASP (Apple Authorized Service Provider) in your local area...

Troubleshooting a Blurry iSight Camera App:
What does PHOTO show? What do you get when you flip the image (far-bottom right)? If all is clean, then it is a bug in the Camera app.

So, what to Try:
A. Disable then Enable Camera:
B. Force Restart the iPhone:
Go Here: Force Restart iPhone - Apple Support
C. Clear the System Data:
When apps are not operating as they should, then you should clear the System Data. It is caches, which are created for apps' remembrance of use, so that they open quicker, and operate more efficiently. Use my User Tip: How to Clear "System Data" in your iPhone's Storage: - User Tip
