Often after an update, the carrier needs to roll out their own update. To come to the conclusion it is not a carrier issue is the wrong conclusion. Contact your carrier who likely needs to re-provision your account. Apple doesn't have anything to do with carrier provided call features. And I'm on iOS 26.2 (the most current version) and have absolutely no issues merging calls.

SMS, MMS, WiFi calls, Call Forwarding, Call Waiting, Call Merge, Conference Calls, hotspots, and Voice Mail (Visual or Normal) are all carrier-specific features. Contact your Carrier (Cellular Service Provider), the agency that issued the SIM / eSIM to you.

If your carrier supports it, you can set up a conference call on your iPhone with up to five people.
The number of participants on a conference call — as well as the ability to add, merge, or disconnect individual callers — depends on your carrier. Contact your carrier for more information about your conference call options.
As this article states:

ever since my update to ios26.2.1 I cannot merge if the original call was an incoming call. I can still merge calls if I am the initiator of the first call. But when I receive a call, and then need to merge it with another call that I am dialing, the merge button appears but doesn't do anything. This has only happened since the update. AND the merge works fine if I place the first call and then add another call to merge, so it is not my carrier settings. VERY FRUSTRATING
It's not always the iOS update's fault! Many things can cause phone problems. If you tell people you think it's an iOS update, it might make it harder to fix or find a solution for the real issue.

Remember, the right question often leads to the solution!

Was/Is your device Jailbroken? If that's the situation, please be informed that attributing the issues to iOS updates may not be accurate.

iOS updates aim to enhance user experience, boost security, and fix bugs rather than disrupt core features. The majority of the issues after updates might stem from underlying, unmentioned factors. Updates won't typically alter personal data but may refine settings or features. Backup before major updates for safety. Review Apple's update details and backup advice for a smooth process.
Kindly consider checking your settings and addressing any device issues before attributing the issue to external factors. This could be a pure coincidence that the iOS update affected the iPhone functions.
Before initiating a standard iOS update, it's advisable to create a backup. Have you completed this step? If yes, proceed with the restoration from the backup.

Which confirms the feature as being carrier-controlled. Call merging requires more cell network resources, why they would not want to give it to you for free.
I am having the same problem. Unfortunately, it’s inconsistent. Sometimes it works and sometimes it doesn’t. When it doesn’t work, I’m on a call with caller A, I add caller B, and once I merge the calls, it shows that they’re both merged but caller B can’t hear anything and doesn’t appear to be there.
I just tried to merge two calls on my iphone and i initiated both the first call and the second. I could hold and add a new number but once i got the second caller on the phone there was no merge option. this is after the update to 26.2.1. Here are some other very important and annoying changes. the turn off on the cell phone alarm is different making me think i am turning off the alarm but instead i am snoozing it. The recents function is now missing from the main phone screen. Actually most things that were handy are now layered in multiple steps using pathing that does not make sense. even the back button does not bring you back but to a useless screen that divides contacts from all, all icloud, and all gmail. why is this screen even necessary? I've been a user for over twenty years and this is my first update complaint. Its like someone without a logical brain cell reinivnted the phone. WHY???????
It wasn’t until this update that I haven’t been able to complete merge calls. So, it is the update and not my carrier.
I cannot merge calls with a toll-free number, but to a USA number I can.
What Call? On an iPhone, you can make and receive various types of calls. Here are the different types:
It's important to note that the availability of certain call types may depend on your iPhone model, iOS version, and carrier.

iOS 26.3 was released by Apple about 24 hours before your post.
