You can remove the flashlight from your lock screen by hard pressing on the lock screen, then tapping Customize, the tap the minus sign on the Flashlight. Before you do that, I'm going to guess you carry your phone in your pocket? If my guess is correct, you are more likely tapping buttons and the screen unintentionally. For the screen you can stop it from unintended touch, by opening Settings > Accessibility > Touch > Turn off Tap or Swipe to Wake. This will stop the screen from opening in your pocket and movement causing things like the flashlight to be accidentally turned on. Going forward, you will need to tap the side power button to wake your phone.

For the Silent Mode, open Settings > Action Button and swipe through the various options till you arrive at None and you won't be able to accidentally mute your phone.

I wasn't aware of any of this. I have no idea how the action thing works or what the concept is. So I have swiped to no action but there is no button to select it.
