Question

No + sign on my wallet app on my iPhone 13.

I am attempting to add my driver's license, passport photo, etc., to my iPhone 13 and have all updates installed on my iPhone. I do not have a + sign on my wallet app and have tried following others' advice to no avail! Any assistance would be much appreciated.

The one time I encountered that, the solution was to go to Settings>General>Language & Region>Region and reselect the U.S. While it appeared to be correctly set, for some reason, it wasn't being recognized.

Are you attempting to access the Wallet app by double clicking the side button or launching the Wallet app from your Home Screen?
What region is set on your device?
Do you have a VPN installed?
Are you on a family plan where your device is managed by an Organizer?
Do you have Screen Time preventing the use of the Wallet at Settings > Screen Time > Content & Privacy Restrictions > Allowed Apps > Wallet?

And I would recommend to post a screenshot of what you see on the top of the screen. There should be no personal information there, but if there is make sure to redact it.
