Unlock iPhone/Watch with forgotten Apple ID, POA for parent

My father has dementia and has been certified as no longer having capacity to make decisions for himself. He is now in permanent care. We have set up an older iPhone with the accessibility settings for him so that he can call us more easily.

I have an enduring Power of Attorney for myself and my Mum - and an iPhone and Apple Watch that we could use, but we don't have his iCloud or Apple ID passwords. They are long forgotten. Is there any way that I can provide these documents and our ID information to Apple so that they can advise how we can unlock these devices and use them on our own Apple ID's?

There's two routes I can think of.

1) This would be easiest if you can find the receipt and also do not care about the account contents. Apple can completely reset a device if a person can provide sufficient evidence of valid ownership. Read the requirements and steps for an Activation Lock support request at: https://al-support.apple.com/#/additional-support and links at the bottom right for the type of service requested. Apple says they may require "proof of purchase documentation” and that you need “proof of ownership documentation”, but does not detail what currently constitutes proof other than it “must include the product serial number, IMEI or MEID.” User reports on this community indicate that typically Apple will ONLY accept an original receipt issued by Apple or an Apple Authorized Reseller. Presumably this is the only kind Apple can authenticate against fraud since Apple is not involved in third party sales. Apple recommends that you "Keep your receipt after you buy an Apple device" --> Keep your receipt after you buy an Apple device - Apple Support

If Apple resets the device this will give you access to the device but won't restore access to the account used to lock it. There is nothing that can be done to expedite an Activation Lock request.

2) Apple provides information to get access to an account for a deceased person at --> How to request access to a deceased family member’s Apple Account - Apple Support

I know this person is still living, but it may be possible to use legal methods in this case too. I have no idea and you would need to speak with somebody at Apple. I suggest using the link in the article since they would be more familiar with what might be required.
