How do I stop preview from adding/altering fields in PDF forms?

I was filling out insurance forms for my two children, in separate Preview windows. Instead of using the info I provided Preview several times altered the info without asking, plugging in data from my own contact card and once changing an address to the address in the other form. How do I stop this behavior? Preview should ONLY fill in fields with explicit permission by the user.
