iOS/iPadOS 26.4+, 26.5 causes Recovery Mode when installing App Store apps or adding Safari websites to Home Screen

Since updating from iOS/iPadOS 26.3.x to iOS/iPadOS 26.4 and later versions, both my iPhone and iPad started entering Recovery Mode / Recovery Assistant after specific reproducible actions.


The issue still happens on iOS/iPadOS 26.5.


Affected devices:

- iPhone 16 Pro

- iPad pro M4 11'

- iOS/iPadOS version: 26.5

- The issue started after iOS/iPadOS 26.4.

- iOS/iPadOS 26.3.x worked normally.


Confirmed triggers:

1. Installing a new app from the App Store.

2. Safari → Share → Add to Home Screen.

3. Opening/checking Beta Updates.

4. Installing apps via Apple Configurator.


Important details:

- Normal update to iPadOS 26.5 completed successfully.

- However, after updating to 26.5, installing a new app from the App Store again caused the iPad to enter Recovery Mode.

- After restoring from backup, using Safari → Share → Add to Home Screen also caused Recovery Mode again.

- Recovery Assistant does not resolve the issue.

- The device usually needs to be restored from backup.


Troubleshooting already tried:

1. Forced restart.

2. Recovery Assistant.

3. Full restore through Finder / Apple Configurator.

4. Restored from backup multiple times.

5. Updated to iOS/iPadOS 26.5.

6. Reset Home Screen Layout.

7. Tested on both iPhone and iPad.


Important observation:

Reset Home Screen Layout temporarily helped once. After resetting the Home Screen layout, I was able to restore previously purchased apps from the App Store and install some apps via Apple Configurator.


However, after manually restoring my previous Home Screen layout, installing a new app from the App Store caused Recovery Mode again.


This makes me think the issue may be related to:

- SpringBoard

- Home Screen layout

- WebClip registration

- IconServices

- MobileInstallation

- App registration after iOS/iPadOS 26.4+


This does not look like a normal App Store issue or a single-device hardware problem, because the same issue happens on both iPhone and iPad with the same triggers.

As a heavy user of iPad (my device of choice for my routine computing needs) I am pleased to report that I have not encountered any of the difficulties that you describe.


So as to eliminate possibility that the trigger is not related to your iPad configuration (or possible corruption of it) - that would be restored from your device backup - as a diagnostic exercise you would be best advised to perform a factory restore. Do not restore your backup - but instead set-up your iPad "as new" with your existing Apple Account.

Having restored your iPad, reinstall Apps from the App Store - and test your iPad. If the problem can not be reproduced, you will have demonstrated that the problem lies with corrupt device/service configuration being restored from your device backup.


If instead the problem can be replicated, your next best step is to seek assistance directly from Apple Support. You can contact the Support Team using the Support link at top-right or bottom-left of this page.

I agree that a clean factory restore without restoring from backup is the correct diagnostic test.


I have now clarified an important point:


On a clean setup, the device works correctly.


The problem started immediately after updating the existing device state from iOS/iPadOS 26.3.x to 26.4. Before 26.4, everything worked normally.


After updating to 26.4, the current device state — not a restored backup at that moment — started showing the issue:

- installing a new App Store app could trigger Recovery Mode;

- Safari → Add to Home Screen could trigger Recovery Mode;

- Beta Updates / Apple Configurator actions could also trigger the issue.


This makes me think the problem may be related to a migration/regression introduced in iOS/iPadOS 26.4, where an existing 26.3.x Home Screen / SpringBoard / WebClip / MobileInstallation state becomes incompatible or corrupted after the update.


Therefore, when restoring from a backup, the problem comes back because the backup effectively restores the same pre-26.4 state/layout/configuration that worked on 26.3.x, but appears to be problematic after iOS/iPadOS 26.4+.


Reset Home Screen Layout temporarily helped once, which also points to a possible issue with Home Screen layout / SpringBoard / WebClip / IconServices / MobileInstallation rather than hardware.


The same behavior happened on both my iPhone and iPad after iOS/iPadOS 26.4+, while both worked normally before on 26.3.x.


So I agree that a clean restore works, but the issue seems to be caused by how iOS/iPadOS 26.4+ migrates or handles the existing device configuration/layout from 26.3.x.

Thank you. That matches my current conclusion.


A clean install of iOS/iPadOS 26.5 appears to work normally. The issue seems to happen only when the existing device state/configuration from iOS/iPadOS 26.3.x is carried forward into 26.4+ or restored from backup.


So it looks like iOS/iPadOS 26.4 introduced a migration/regression issue affecting certain existing configurations, possibly involving Home Screen layout, SpringBoard, WebClips, IconServices, MobileInstallation, or installed app state.


Reset Home Screen Layout temporarily helped once, which further suggests the problem is related to the existing layout/configuration rather than the hardware.


I understand that rebuilding from a clean install may be the only practical workaround for now, but I hope Apple can investigate the migration issue because restoring from backup brings the problem back.

iPadOS 26.4.x introduced a number of significant issues of which I am aware - and no doubt others that have been encountered with specific combinations of configuration or installed Apps, such as your own experience.


iPadOS 26.5 has, thus far, been both stable and smooth running subsequent to installation of the update. If you rebuild your iPad from clean install, whilst an unwelcome task, you will hopefully restore nominal operation and performance.
