It turned out that the firewall was blocking the connection. I opened the Firewall from the MacBook Settings. The Firewall was toggled On. I clicked Options and found that ContinuityCaptureAgent was set to "Blocking Incoming connections". I changed it to "Allow Incoming connections". After this one change, continuity camera started working for me and I can see my iPhone back camera from my OBS.

I have never recommended the computer side application firewall be turned on...
it is not turned on by default.
