Photo scanning errors: out of focus or too dark
Trying to scan certain photo keeps saying out of focus too dark try again not enough light etc

I've found that it's easy to try to be too close. I'd suggest that you back off some. That may also allow more light in.
More light allows the camera to use a faster shutter speed and reduce image blur caused by motion, especially if the iPad is handheld.
