This may sound self-evident but have you checked your iPhone settings? If you go to Settings > Phone > Calls on Other Devices, you'll see a global setting or your other devices capable of showing calls (Mac, Apple TV etc.) you'll have to enable that.

Alternatively on your Mac open the Phone App (macOS 26 and later), go to settings (Phone on the menu bar > Settings or just press cmd+,) go to the Calls tab and see if "calls from iPhone" is enabled.
