Do you have attention aware features turned on --> Turn Attention Aware features on or off on your iPhone or iPad Pro - Apple Support
Also, you have an iPhone which was released in 2021, 5 years ago. If your phone has the original battery it was likely time to replace the battery some time ago. Open Settings > Battery > Battery Health - what does it show there? If 80% or lower, the battery is failing and needs to be replaced and failing batteries often cause issues with volume.

So sad I haven’t been able to see any solutions for this. Having came from a Samsung S25 (which got spoilt due to screen issues) and now I am using the iPhone 12 for the main
one consistent issue I have faced is the phones volume randomly reducing when battery is low, when I am playing FPS games, and when unlocking my phone with Face ID my music volume reduces.
now before any answers with the obvious solutions, I assure you I have done all of that.
so please I need a real solution cause it’s frustrating.

The Solution is to not let the battery run down so much. Your phone is simply protecting itself when the battery is lower than 20% which your screenshot clearly shows it has to be. Plug your phone in to charge when you go to bed at night and leave it plugged in ALL night, EVERY night. Check your battery health as an iPhone 12 can be as old as 6 years old at this point and if it has the original battery in it, it likely needed to be replaced long ago. In Settings > Battery > Battery Health - if the health is displayed at 80% or lower, the battery is failing and needs to be replaced.
