iPhone 14 Accessibility Reader not reading notifications

My blind brother uses the accessibility menu constantly. His favorite accessibility feature is for his iPhone to read out load the notifications on his home screen. He says this feature used to work flawlessly on IOS 18 but is not very reliable on IOS 26.5.2.

His phone is an iPhone 14, running IOS 26.5.2. He is using the Accessibility Reader feature. He has the feature assigned to a shortcut to make it easy for him to activate it as needed. The issue is that his phone will report that the contents of the screen cannot be read. His wife then checks his phone and finds lots of Notifications. The feature does work on text messages and other contents but not on the Notifications like it used to.

He has tried many things to correct this situation but failed. To me it looks like a bug given that it worked on an earlier IOS release and now it doesn't.

Any help out there?
