I don't disagree with you and unfortunately my Doctor does tell me to contact my Insurance company constantly for covered services and discrepancies on what is billed and paid by my insurance. I wish it wasn't that way either.
The carriers do provide their own Carrier Updates on your device that they push out for features and provisioning that are independent of the OS updates. You can manually check if a Carrier Update is available and also view the version number of the one installed on your device where the carrier will be able to tell you if it is the most recent one. This may also be required if you set up a new eSim or install another SIM card on your device.

I also have T-Mobile and frankly it is their job to provide the mobile service to your device, with all of the supported features. I do have the MMS Setting option with T-Mobile. It is a cop out for them to say it is not their problem, so it would be Apple's fault. Their service would be severely impacted if every iPhone was not able to use it, and you can bet that is not the case. With other cellular providers offering you money if you would switch to them, you would think they would take this matter much more seriously. I am not here to bash T-Mobile as I have not had any problems with them, but in your case they need to step it up.
