If it's for a physical SIM then it might be written on the plastic card that the SIM came in. If you you need to call your carrier and ask them. No one here or at Apple can help you. Only the carrier

If your SIM card is requesting a PUK code, only your cellular service provider can give that to you. Nobody here in these user to user forums or even Apple can magically produce a PUK code. Don't try to guess it. Call your mobile company and request it from them.

If you don't have your PUK code anymore, you need to contact your carrier. We cannot help you unlock your simcard
In addition to the excellent replies you have received, if you had an online account with the carrier you can log in to their portal and one of the options should be to view the PUK code.
