If you are asking why the mai tho ้ is highlighted instead of the mai ek ่ when you press the special key, this is presumably a bug. Are you able to make mai tho when you want to? What iOS are you running.
It doesn't behave like an ordinary letter. iOS may display a dotted circle when you select the mark by itself because the keyboard is showing where the mark will attach.

You should delete this. The question is about Thai and not Devanagari and has nothing to do with dotted circles but concerns the behavior of a particular Thai keyboard, which you do not seem to be familiar with.
