Although Microsoft Word has this capability, it is not built into Pages. All you can do short of scripting is click the document name in the document title, and copy/paste into the footer.

For the path, you can use a short AppleScript to put the full file path on the clipboard, and then click in the footer to paste that path there. The following AppleScript assumes you have a Pages document open in Pages:

tell application "Pages"
	tell front document
		set thisPath to its file
		set the clipboard to (POSIX path of thisPath) as text
	end tell
end tell
return

Copy/Paste that script into Apple's Script Editor, click the hammer icon to compile it, and then click run. There will be no output from the script, but you will see the file path from Finder > Edit menu > Show Clipboard.

There are more complicated scripts that can write that path string directly where you want it in the document.

If you'd rather not run a script, Finder can copy the path for you. Find the document in Finder and right-click it, then hold down the Option key: "Copy" changes to "Copy … as Pathname". Click that (or select the file and press Option-Command-C), then click in the Pages footer and paste with Command-V. It goes in as plain text, so if you move or rename the file later the footer won't change by itself, you'd just paste the new path in.
