Just like you said, the iPodVoiceOver.dll file had installed itself to the root directory of my C: drive. I was able to drag-and-drop it into the C:\Program Files\iTunes folder, after which I could successfully enable VoiceOver on the Summary tab in iTunes for my iPod Shuffle.
I likewise run 64-bit Windows 7 and have my iTunes library on an external harddrive, so perhaps this is the common thread for those of us experiencing this bug.
