Apple repair service??

You should take it to an apple store

If you can't get to an Apple store, Apple will send an empty box to your house so you can ship your product to their repair center.

In my experience they do not repair they give you a replacement warranty replacement iP4 ( ie NOT a new boxed retail item.)
However at early life the warranty replacements are more than likely still new items.
All problem iphones are refurbished at an Apple manufacturing facility
As advised make a Genius bar appointment at an Apple retail store for best service .Usually
replaced on spot
recognising issue with being on IoM do you have an Apple store there

I did this out of warranty. They will send you to the nearest ups store if you live in the US. You will drop off your phone and they will overnight it to apple. Apple inspects the phone to make sure you are having the issue you claimed and that it's not a dropped or dunked phone (unless of course your paying for an out of warranty at $215 roughly) and the next day ship you back a replacement phone. Plug it into its and restore and it's like you never got a new phone at all neverythjng is supposed to there. A few exceptions is passwords like wi-fi and RSA key generating apps (blizzard authenticator for one) no passwords and the key generator apps have a new serial number. If you don't know what I am talking about then likely you won't be effected further than reading all your wi-fi passwords.

Why don't you just call Apple Care and tell them your situation including that you DO NOT have an Apple store near you and get the propper procedure and get the ball rolling.

Nothing other than the phone is necessary, although the instructions should either be emailed to you or sent in the box they provide...

I believe if you do the advanced replacement by paying 29.99 they will send you a replacment phone, and they will also include the white box so that upon receiving your replacement, you put your old one in the white box and send the phone. I believe they will charge you 599 on your credit card account for the time being but once they receive your defected iphone/broken within 10 days, the 599 will no longer be held and pretty much you get the 599 back. You don't need to send the cable or anything, just the phone itself.
