I had that problem after updating to iOS4.1. Investigation showed the problem to be the plastic covering the sensor in my Otterbox Defender. Removing the plastic solved the problem for me. So if you have your phone in a case which uses plastic to 'protect' the display you might try removing the case and if the problem goes away then you don't have a sensor problem. If you aren't using a case then you probably do have a sensor problem.

Try eliminating other possible causes, like a case that casts a shadow on the proximity sensor. See if you still have the problem if you remove the case.

Some things people have suggested is to do a restore as a "new" phone. Kind of a pain, but some say it helps.

Some say reset network settings. Some say reset all settings.

Also a hard reset.

If no luck, there is a free app workaround call "Call Guard". You make your call, press the home button to go back to your app screen, launch Call Guard. It brings up an almost blank blue screen - thus no buttons to activate by accident. You get out of it by pressing the home button again or you can press the power button on top which will exit the app AND end your call in one step.

One nice thing about it is that if your screen is letting you accidentally activate buttons, Call Guard shows how many times it happened AND where each tap occurred. Handy to actually see what is happening.

When on the phone, do you tend to move it around much (you might not normally think about it)? There is something many of us have discovered that involves sudden changes in position which makes the sensor act up.
