iPhone 4 - Error - Fixed Number Dialing Activated

Apple replaced my iPhone 4 recently. After the replacement the phone displayed a message on the screen that reads:

Fixed Number Dialing Activated

Neither Apple or AT&T knows that the @$@#$ that means. Ugh. AT&T's solution was to replace the SIM card, but upon replacing the iPhone 4 displayed the message again.

The phone had problems after activating and displaying this message where as it would display No Service, fail to make or dial any calls even to customer service, etc. This would happen. I'd reset it and it would happen again. Finally today it seems to be okay, but the intermittent behavior is a big problem for me.

Anyone encounter this?

iMac Extreme, Mini 2.0ghz, MBPro13 2009, Mac OS X (10.6.4), iPhone 4 iOS 4.1

Fixed Dialing Number (FDN) is a service mode of a GSM phone's SIM card. Numbers are added to the FDN list, and when activated, FDN restricts outgoing calls to only those numbers listed, or to numbers with certain prefixes. Not all SIM cards support this feature, a PIN must be entered beforehand, which is unique to each SIM card and provided by the network operator. The PIN to enable this is separate from the PIN to lock or unlock the SIM. Many parents use this to restrict calling on phones given to children. This restriction has no affect on incoming calls. I'm surprised AT&T was clueless since they're the one's that have to provide this feature.
