Odd keyboard shortcut problem

I've created a custom keyboard shortcut for a preset I created in AP3.1. The shortcut works great on the first image but when I select the next image the shortcut does not work and the Photo option in the menu bar flashes. It is not a duplicate shortcut. When the shortcut works the Photo option does not flash. I've changed the shortcut but problem persists.
If i use the preset from the preset window it works every time. I have created other shortcuts and they are working but those were created in AP3.0. Not saying that 3.1 is the issue.
