Fix #1
Delete the iPhoto preference file, com.apple.iPhoto.plist, that resides in your User/Library/Preferences folder.
NOTE: If you're moved your library from its default location in your Home/Pictures folder you will have to point iPhoto to its new location when you next open iPhoto by holding the the Option key. You'll also have to reset the iPhoto's various preferences.
Fix #2
Launch iPhoto with the Command+Option keys depressed and follow the instructions to rebuild the library. Select options #1, #2 and #6.

The same thing happened to me!!! I lost my back page and flap in a photo book, and there seemed to be absolutely no way to recreate them. I had spent HOURS (days? weeks??) on that photo book, and didn't like the thought of recreating it from scratch!!! Thank you very much cohnfamily for the tip!!! 😀

Genius! At first it wouldn't work but the trick was to insert a double page in spread layout into the two last pages. You saved my hours, hours of work...

I'm almost done with a huge freelance project, and boom! Missing back flap and magicly apearing pages that pushed my book into 105 pages!? Yeah, wierd.
Anyway, Hokey fix all the way! The extra "mystery pages" were easy, but that back flap was freaking me out!!!
