Hi there. Tried turning the bar all the way to the right. I can hear the ring tone but it still says silent mode and there are no key clicks and I can't hear the person on the phone but they can hear me.
When you say it stays in silent mode what is it doing that makes you think it is in silent mode?
First, make sure the switch on the phone is set so you cannot see red behind the switch. A bell will appear on the screen (crossed out bell means silent mode).
Make sure "keyboard clicks" are set to On in the same sounds settings screen.
Not being able to hear the person on the call is an entirely separate issue as silent mode only relates to alert sounds, not the audio coming from a telephone call. Are you sure they are not on hold?
In my first message I noted that the button on the left is not showing red it is in the correct position. When you go to the "sounds" at the very top it says "silent". The keyclicks are on but you can't hear anything. So it is indeed in silent mode despite the fact that all the settings are set properly.
In regards to the button I was making sure as sometime users get confused which toggle setting is which. It is also often helpful to toggle it back and forth because sometimes the software missteps and thinks it is in a different position that it is.
In sounds, Silent will be at the top as it is the first menu item. This just allows you to change the vibration setting. If you can still hear a ringtone when adjusting the slider it means your speaker is working.
At this point I would recommend you [restore the phone|http://support.apple.com/kb/ht1414] to factory settings and start over because something is clearly not right.
I'm having the exact same issue. My iPhone 4 worked fine one moment, shifted to silent mode the next and now can't be made to ring aloud again. No keyclicks, either. I'm able to hear the ringing in my bluetooth headset, but that's it.

I too am having the same issue. One minute all is well. The next it's silent. iPod sound doesn't work either. Have to reboot the phone to get it to work again. Obviously there's an issue. A very annoying one.
Brought it in to the Genius Bar and they promptly replaced the phone. The fellow said that he's seen the problem before (not frequently) and it's definitely a hardware problem. You have to try restoring the phone and setting it up as a new phone to insure that it's not an app that's messing things up, but once you do that and if the phone still doesn't work, it's a hardware issue.
