I found out where the missing icon was!!!
I have most of my Apps in folders. Most of the folders only have a few apps in them. Even though there are only a few apps on the first page of the folder some folders have a second or third page. The unseen icon is there. Because only a few apps were in the folder it didn't enter my mind to look in the second page of an almost empty folder.

Same thing happened to me and I was sure I had synced the app to my phone. I could find it by searching but it definitely wasn't anywhere as an icon, alone or in a folder. Finally thought to check how many pages of icons I had haphazardly added (easiest to see in iTunes> your phone's name when connected > apps, since they number the pages) and I had reached the 11 page limit. I organized most of my apps into folders and reduced down to 6 pages .... Then had to reboot and lo and behold, there it was - along with half a page of icons for other apps that I had forgotten about and weren't showing up before! 🙂 This is a hazard when you keep picking up games and apps when they go free for a while. I try to only keep a few on my phone (unchecking the rest in itunes>my phone when connected > apps) so I can test them out gradually, but it got away from me rather easily over the past year. I now have two folders full of "task lists" to try out, for instance! You can see I really do desperately need a proper task list, if I ever find time to find the right one. (Catch-22.)
Apple should incorporate a warning message when we reach that magic 11 page limit, at least ... I like to be told by my gadgets when I'm about to do something stupid.
I just had a similar issue - cd not find the
App for the app store on my iphone
which was recently repaired.
I realized in Settings - the app was
on the menu tho it had been turned
off.
I set it to 'on' & found the icon in a
folder on my iphone's home page.

Same happened to me.
You've reached the "11-page limit"...
Don't panic , although the apps are still present, they cannot be displayed.
Fix it by:
1. Merging apps into folders
2. Power off phone
3. Power on phone - the 'missing' apps should now show up.
4. Repeat if necessary

Go to settings and reset your phones app layout to the original placement. I've had the same issue although I had most certainly not reached the 11 page limit. This has happened two or three times and after resetting the layout it reappears. Btw, the only time this has occured is when I have placed an app into a folder.
