uneven audio balance

I have an issue with audio balance. The left side is slightly lower in volume no matter which output I select.......internal, external firewire etc. I have reset pram etc. I make my living making music and this is making it impossible to work and get an accurate idea of my mixes. Any help would be very much appreciated!!

Mac pro, Mac OS X (10.6.5)

Adjust the balance slider in System Preferences... -> Sound -> Output.

Yes, I have done that. The problem still exists both on the line in/out port as well as my pro tools and digital performer interfaces. Does anyone know if there is a designated audio card/chip that could be failing? I've talked to several knowledgeable people and they are baffled as well. I have checked this with headphones as well to make sure it is not a speaker or power amp issue.
