As you can tell, I've been banging away at this for a little while. It's not working. When the shift+restores fail to update, I get white screened on my iPhone 3GS. I can't bring my phone back to the original settings, I can't update it.

I've restarted my computer in here a few times, switched USBs etc. Any advice as to what I'm doing wrong?

*EDIT*
Note, most of these errors occur on verify with Apple, the shift+restore to 4.2.1 fails while preparing.

The 3194 error appears to have something to do with the digital signature of the OS file that you downloaded. I found this "fix" that people use to downgrade their OS, no reason why it shouldn't work for the upgrade too.

* 1) First navigate to “c:\windows\system32\drivers\etc\” and open the ‘hosts’ file in notepad. To do so, right click on the file and select Open With->Notepad. At the end of the document, add this string “74.208.10.249 gs.apple.com”.
* 2) Now connect your iPhone to iTunes in recovery mode. To do so, first switch off your device. Then press the Home button on your handset and connect the USB cable. As soon as you connect the USB cable, the device will boot up and a “Connect To iTunes” screen will be displayed. Users can now stop pressing the Home button.
* 3) Fire up iTunes now. You will see your iPhone/iPod Touch detected in recovery mode. Now, whilst pressing the Shift key for Windows users or Alt key on the Mac platform, click on the Restore option.
* Now just select the original OS x.x.x and voila, you are done. Now you can again enjoy OS x.x.x on your iTouch device.

I did this. Error 1601. Currently, I've downloaded iREB, followed the steps and used it, and am trying to shift+restore to 4.2.1, but it's taking just as long as the normal Error 1601.

*EDIT*
Confirmed Error 1601 on the failbreak recovery.

Current steps I'm taking: Apple suggest uninstall (Again).
1. iTunes
2. Quicktime
3. Apple Software Update

5. Bonjour

I found this for the first error you listed, 1403:

"This error means that your ipsw file is corrupted. If you downloaded it from the net, try downloading it again."

"To check if it is corrupted, rename the extension to .zip and open it with winzip.
If it doesn't open/extract, it is corrupted."

It looks like a corrupted .ipsw file started the whole nightmare process you are going through. Hopefully something in one of the posts I've made will work or someone with more knowledge will join in and help you get your iPhone running again.

And now I'm more confused than a chameleon in a bag of skittles.

I went to shift+restore to 3.1.2. Error 3194.

And my 3GS turned back on. So lost...

Well now that it is unbricked, I need to figure out how to bring it to 4.2.1

*EDIT*
Oh and I can't plug it in with iTunes because the Apple Mobile Device service isn't running? Easily fixed in services, but still annoying...

"Oh and I can't plug it in with iTunes because the Apple Mobile Device service isn't running?"

I can help with that. Right click the My Computer icon on your desktop and select manage. Expand Services and Applications. Select Services and a list of all services will appear in the right side pane. Right click the Apple Mobile Device service select start. The service should start running.
