My Dual bootable, DP MDD 1.25GHz G4 (2004) came with AHT version 2.0.2. The DVD, labeled version 1.0, comes with Panther 10.3.2 and the part number is 691-4872-A.
It yields a 2.68 GB disk image, which could be burned onto a new DVD. Its size exceeds what can be moved through yousendit.com for free. Perhaps you can find something similiar online?

Sep 7, 2012 6:40 AM in response to Peter Treutby Nick Baldwin,Confirming that the downloaded AHT 1.2.7 is working here on a Mirror Door 2003 1.25GHz single processor, with the original discs missing. AHT 1.2.6 is not working and 2.0.1 is not supported.

Attempt 1: It just booted into Tiger.
Attempt 2: I got a lovely kernel panic.
Attempt 3: Booted into Tiger again.

- Open Disk Utility, in the Utilities folder (/Applications/Utilities).
- If the disk image you want to use doesn't appear in the list, drag its icon to the Disk Utility window.
- Select the disk image and click Burn.
- Insert a blank CD or DVD into your computer's optical drive and follow the onscreen prompts.
