I had it on mine too. When I went to exchange the phone for a new Iphone at the app store, the new iphone did not have facetime on the homescreen.

That's because the iPhone has never had a FaceTime icon on the homescreen.

Plus, I have a screen shot of the iPhone 4 which shows the FaceTime icon on the app screen. I'll see if I can cut and paste in later. When I upgraded to iOS 5 the face time app icon vanished, along with a lot of other things. I really regret updating.

This is not referring to a separate app. While you are on a phone call, there is a FaceTime icon on the screen, along with the speaker, mute, dial pad, contact and add call icons. Make a regular phone call and you'll see what I mean.

To simplify what everyone else is saying:

The FaceTime icon is only on the home screen of the iPod touch. It has never been on the iPhone because you access it either through Phone or Contacts.

If you are talking about the facetime app on homescreen. Then you are talking about ipod touch. iPhone does not have Facetime App on the homescreen and it was never there since the iPhone 4. Go to Settings > Facetime, make sure it's on. If it is on, all you need to do is go to your contacts and you should have Facetime button there to start facetime. Or while on a phone call you can tap facetime from there.
