Ok, a real novice here. I've made a spreadsheet and want to create a form for it. I read that I need a header form. What, where and how?

you need a table with a header row filled out. then add a new sheet and you will be given the option of creating either a form or a new sheet. If you have two or more tables with header rows, you will be given a choice of which table to base your form off of.
Make sure you pre format your data columns in your table so that the form shows the correct keyboard and choices when you tap into the fields.
