The hard way is to print out the document, sign it, then scan it to a PDF, and finally email it back.
The easy way is to first scan in your signature. Then you can open a received document, paste in your signature, and return it via email.
In my case I made several signatures on a blank sheet of paper, scanned in the sheet, then used Graphic Converter to select the best exemplar and saved it to a separate file as a JPEG image. When I receive emailed documents requiring a signature I have them sent as PDFs. I then use a third-party application, PDFPenPro, to edit the PDF and to paste in my signature. I save the edited PDF and return it via email.

Within Preview, once you have your signature scanned and saved, it is actually pretty intuitive once you've chosen the signature tool and click on the line where you need to sign. You can always adjust the size of the signature if it doesn't look appropriate.
My tip for your initial signature scan is to exaggerate it's size on a single sheet of paper. For me, that made it easier to hold it in front of the camera and easier for it to detect.
I have done it and while it worked it was difficult and clunky.

My feeling exactly. Since an electronic sig needs to be as much like a regular sig, and controllable with respect to size relative to the ability to size to even short lines and small spaces, and have a perfectly white or transparent background, I was very disappointed with Preview when I tried it out.
Have you ever tried it? I did, and I'll stick to scanning. Preview means well, but the execution is terrible.
Years ago before iPhone cameras, PhotoBooth, or even OS X, for that matter, there were scanners. I'm still using those scanned signatures. 🙂
