You read carefully chapter #11 in Pages User Guide.
In the English version it means pages 243…249 so I can't resume them here.
PS: it would be a good idea to update from 10.4.6 to 10.4.11

My apologies, you are right Jerry.
I experimented with Layout on the off chance this was another of those gotchas, and Insert > Merge Field was greyed out.
This time it took.

May I repeat that 10.4.6 isn't a valid Operating System version to use iWork '09 ?
So, if the OP is really using this old version it may explain the described behavior.

I just tried it and it worked. I'm used to the MS Word order of doing this. It seems a bit weird and backward to put the fields in before telling the file that it is going to use mail merge.
As for Mr. Koenig's answer, The problem was not i the operating system, but in the sequence.

Difficult to guess that you didn't did the required tasks without applying the directions given in the User Guide.
There is no reason to assume that the MS Word requirements apply to iWork.
