Open a free Skype account. The app will allow you to call another computer or device with the app installed for free. In other words, you can call a skype user name for free.
There is a paid account though if you want to call a land line at a home or business. You purchase Skype minutes to do that.
I have found calling an iPhone from my Mac using Skype is clearer than using AT&T. There is also video support if they have a camera on their machine or device. It can be very good quality!

Also, regarding sound quality, I'd recommend getting an iPhone headphone set to use with it. It contains a mic and volume clicker right on the lanyard. That will help eliminate background noise and give you a good experience.

Yes. Microsoft now owns Skype. Before that, EBay was the majority stockholder, I think.
And I don't see any reason why a newer Bluetooth device should not work. But it has to support the newer A2DP spec is my understanding.

I know. They need an update. Maybe once iOS 5 rolls out. But then again, now it's owned by MS. Who knows.

That may be the answer for iPad, iPhone Skype calling on Bluetooth.
The best way is Skype. It's free and it works fairly well. There may be some lag, but it is bareable.
A deal for Mac users... buy Microsoft Office 365 (install on 5 computers) for a year and get 1 hr of free Skype every month = free phone (and you get office for $100). Awesome. Good deal if you use Skype. FYI
