It sounds like the cord has a bad connection and needs to be replaced. Go to an Apple Store or AASP and have it checked out.
As rkauffman 87 suggests, the cords can become frayed after a time. Look particularly where the cable enters the body of the power supply.
In addition, where the connector links with the computer, there are 5 pins. The centre one may need a bit of encouragement: try pressing it with a pencil, gently.
Also move the connector around as it touches the port on the notebook: connecting and disconnecting, up and down, a few times. Also turn that part round (so the left side goes to the right), making a different link may work. Other than that, it is off to the shop.
Reset the SMC, it controls the charging process.
